Exosomes vs. Stem Cells: Understanding the Difference

Stem cells are living cells
Mesenchymal stem cells (MSCs) are living cells capable of dividing and differentiating. Working with cells involves considerations around viability, storage, and handling that come with any living biological material.
Exosomes are cell-free vesicles
MSC-derived exosomes are not cells. They are small (30–150 nm), cell-free extracellular vesicles secreted by cells, carrying proteins, lipids, and RNA. Because they are non-replicating and cell-free, they are studied as carriers of molecular signals rather than as living material — a fundamental distinction from cell-based approaches.
Why the distinction matters
The difference affects how each is characterized, stored, and studied. A cell-free exosome preparation is defined by particle measurements and a Certificate of Analysis, not by cell viability. Conflating the two leads to confusion about what a given material actually is.
