The key distinction: cell-free vs. cellular
MSCs are living cells. MSC-derived exosomes are the cell-free signaling vesicles those cells release — there are no cells in the final material. A central research question driving the field is how much of what MSCs appear to “do” is carried by these released signals rather than the cells themselves.

Mesenchymal stem cells are multipotent cells studied across regenerative research. Because they are living cells, they are characterized by viability, identity, and potency, and cell therapies sit under strict regulatory oversight.
What MSC-derived exosomes are
